Become a part of our caring community The Care Management Support Assistant 2 contributes to administration of care management. You will provide non-clinical support to the assessment and evaluation of members' needs. This support helps achieve and maintain an optimal wellness state by guiding members/families toward resources appropriate for their care and wellbeing. Additionally, you will triage different risk assessments and assign them to the care management team. You will also follow up with the team inbox and direct referrals to the appropriate queue or tasks those to a care manager. Furthermore, you will complete health risk assessments to identify needs, which will be assigned to the appropriate care management. Be comfortable with high volume of calls and perform outreach to members on a regular basis. You will also assist members by helping them understand their benefits and connect them to resources available.You will report to a Manager, Care Management.Key Responsibilities:Perform outreach calls (around 40 calls per day).Receive/respond to incoming calls or team ques from referral sources or members, triage information to identify patient needs, and determine our ability to meet them.Record the outcome of calls and follow-up on tasks.Distribute inquires to the appropriate care manager.Identify alternative community service resources when solutions are not available.Perform health risk assessment telephonic screening to determine health risk appropriateness for case management.Use your skills to make an impact  Required Qualifications1 or more years of customer service or call center experienceKnowledge of Microsoft Word, Excel and OutlookExcellent communication skillsPreferred QualificationsAssociate's DegreeResides in South CarolinaExperience with medical terminologyFamiliarity with Medicaid populationAdministration support experience in a healthcare industryFamiliarity with care and well-being resourcesAdditional InformationWork-At-Home Requirements:To ensure Home or Hybrid Home/Office associates’ ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office associates must meet the following criteria: ·       At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is recommended; wireless, wired cable or DSL connection is suggested.·       Satellite, cellular and microwave connection can be used only if approved by leadership.·       Associates who live and work from Home in the state of California, Illinois, Montana, or South Dakota will be provided a bi-weekly payment for their internet expense.·       Humana will provide Home or Hybrid Home/Office associates with telephone equipment appropriate to meet the business requirements for their position/job.·       Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information. Interview Format:As part of our hiring process for this opportunity, we will be using exciting interviewing technology called HireVue to enhance our hiring and decision-making ability.
