As a Analyst Care Planner Behavioral Health, you’ll play a key role in engaging members, coordinating care, and helping improve outcomes through education and ongoing support.What You’ll DoConduct outreach to members to promote behavioral health programs and encourage participationSupport members in locating services and accessing community resourcesProvide education on mental health, substance abuse benefits, and care optionsTrack patient progress and treatment adherencePartner closely with clinicians to help drive positive health outcomesHandle administrative support including care coordination tasks, provider research, and documentationWhat You Bring - REQUIRED2+ years of behavioral health care experienceStrong customer service and communication skillsComfortable navigating multiple systems and working on the phoneAbility to manage tasks independently in a remote environmentNice to HaveManaged care experienceBackground in psychology or social servicesEducationAssociate’s degree or equivalent combination of education and experience💡 Why Join Us?Work from home while making a meaningful impact on members’ lives.
