Employment Type:Full timeShift:Day ShiftDescription:ED Patient Case Manager RN (Onsite)Shift/Hours:-Full Time DAY and NIGHT shifts available-Mix of 12 hour and 8 hour shifts available (FLEXIBLE)Position Summary:MercyOne Trinity is seeking a compassionate and detail-oriented Registered Nurse (RN) Care Coordinator to support patients across the care continuum. In this role, you will collaborate with interdisciplinary teams to develop, implement, and monitor individualized care plans that promote quality outcomes, patient safety, and efficient resource utilization.Key Responsibilities:Coordinate patient care plans across inpatient, outpatient, and post-acute settingsCollaborate with physicians, nurses, and community partners to ensure seamless transitions of careConduct patient assessments, identify needs, and develop care plansFacilitate discharge planning and ensure appropriate follow-up servicesEducate patients and families on treatment plans and available resourcesMonitor patient progress and adjust care plans as neededSupport quality improvement, regulatory compliance, and documentation standardsQualifications:Current Registered Nurse (RN) license in IowaBachelor of Science in Nursing (BSN) preferred2+ years of clinical nursing experience (care coordination, case management, or acute care preferred)Strong communication, critical thinking, and organizational skillsExperience with care management systems and interdisciplinary collaborationWhy MercyOne?Mission-driven, patient-centered cultureCollaborative team environmentOpportunities for professional growth and developmentCompetitive compensation and comprehensive benefitsOur Commitment Rooted in our Mission and Core Values, we honor the dignity of every person and recognize the unique perspectives, experiences, and talents each colleague brings.
