They are dedicated to patient and family-centered care that values personal self-determination, skilled at managing multiple priorities at once, and engaging in creative, compassionate, and ethical problem-solving. As a member of a multidisciplinary health team, they perform comprehensive assessments and develop treatment plans that integrate the medical, social, and resource issues that impact individual patients, families, and at-risk populations. Social Workers are responsible for working collaboratively with all healthcare team members to develop and implement treatment plans that support patient-centered plan of care for both individual patients and the medical community.Does this position require Patient Care?YesEssential Functions-Provides psychosocial assessments of patients and families with social, emotional, interpersonal and/or environmental issues. -Formulates biopsychosocial assessment, disposition, and treatment plans. -Collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team. -Monitor, evaluate, and record client progress according to measurable goals described in the treatment and care plan. QualificationsJob DescriptionFull time, outpatient therapist opportunity embedded within a primary care pediatric clinic. Our LICSW will work as a member of the pediatric primary care department to provide short term therapy to patients with clinical mental health conditions including but not exclusive of anxiety, depression, and other mood related conditions as well as ADHD and behavioral dysregulation.  Conducts psychosocial assessments, provides clinical services to pediatric patients that address psychosocial, environmental, age specific and cultural issues in order to maximize emotional, social and physical well-being and the effective/efficient use of health care and community resources.  LICSW will be available to Pediatricians for consultations and acute assessments during routine clinic encounters.Experience and training with elementary aged (5-11) children as well as CBT preferred. Works effectively as part of an interdisciplinary health care team and is comfortable referring to and collaborating with interdepartmental specialists and programs, outside agencies, and other relevant community resources.Assists with guardianship process for patients within the broader Mass General Brigham Pediatric Primary Care practices.Patient CareProviding short-term, evidence-based psychotherapy (individual, groups) to patients in the pediatric practice in-person and virtuallyScreen patients and develop comprehensive assessments and treatment plans for patients engaged in BH treatmentProviding a range of interventions to support primary care mental health and behavioral needs such as crisis intervention; care coordination; information; referrals; and safety planning.Assessing  patients presenting with new mental health and behavioral symptoms to aid in the identification of the best treatment options for themParticipating in formal and informal case reviews, seminars and / or inter- and intra-departmental peer review meetingsWorking closely with the other members from the primary care practice to provide timely and appropriate careAdvocate for school related needs as applicable to behavioral health diagnoses.GuardianshipConduct Medical Certificate for  pediatric primary care patients in the division of primary care to contribute to the guardian process.Collaborate with clinical care team members, iCMP and others coordinating guardianship for pediatric primary care patients.EducationMaster's Degree Social Work required or Master's Degree Mental Health & Behavioral Medicine requiredCan this role accept experience in lieu of a degree?NoLicenses and CredentialsLicensed Independent Clinical Social Worker [LICSW - State License]ExperienceBilingual skills preferred (Spanish or Arabic)Clinical Experience in a medical setting 2-3 years requiredKnowledge, Skills and Abilities- Demonstrates a mastery of a range of specialized clinical diagnoses and interventions.- Conducts clinically competent complex and high-risk assessments, treatment plans, and interventions.- Demonstrates mastery in problem-solving abilities, conflict resolution, efficiency, and communication skills.- Formulates advanced psychosocial assessments and treatment plans.- Supports other staff in developing assessments and plans in complex cases.- Demonstrates advanced clinical judgment and innovation in advocating for clients.- Demonstrates advanced working knowledge of internal and external resources and refers appropriately.- Demonstrate ability to identify problems in order of urgency and initiate treatment plans towards timely solutions and appropriate time management skills.- Demonstrates the ability to articulate a social work clinical perspective in team meetings and effectively collaborates with the interdisciplinary team to comprehensively problem-solve and optimize a plan of care.- 1Creates content for skill-based groups.- Promotes timely and concise collaboration among all disciplines and outside stakeholders to enhance patient care (including but not limited to the Ethi
