Role Description:Oak Street Health takes a team-based approach to providing outstanding patient care. Transitional Care Coordinators are an integral part of the team. You will help track patients across hospital and post-acute settings and help them get connected back to primary care. You will efficiently juggle many tasks: outbound phone calls to facilities and patients, managing medical records, and scheduling post-discharge appointments. We also expect you to be highly flexible, covering additional responsibilities, as needed. This will be a remote work position.Core Responsibilities:Manage inbound notifications related to transitional care events from payer, vendor, facility, and other sourcesConduct outreach to inpatient and post-acute facilities to obtain information, such as admission & discharge dates and discharge plansRequest and obtain medical records, routing documents to clinical staff for review, and uploading documents into electronic medical record platformConduct outreach to patients to schedule and confirm post-discharge appointments within the electronic medical records platformCoordinate with care team, facility, payer, and community-based organizations to ensure patients' transitional care needs are metOther duties as assigned What are we looking for?Strong computer skills, including basic knowledge of Microsoft Excel and electronic medical recordsExperience in customer service setting (specific experience in healthcare preferred but not required)Professional phone etiquetteHigh level of integrityProficient PC skillsCollege degree preferred, high school diploma requiredFluency in Spanish, Polish, Russian, or other languages spoken within the communities we serve (where necessary)US work authorizationSomeone who embodies being "Oaky"What does being "Oaky" look like?Radiating positive energyAssuming good intentionsCreating an unmatched patient experienceDriving clinical excellenceTaking ownership and delivering resultsBeing scrappy Why Oak Street?Oak Street Health offers our coworkers the opportunity to be at the forefront of a revolution in healthcare, as well as:Collaborative and energetic cultureFast-paced and innovative environmentCompetitive benefits including paid vacation and sick time, generous 401K match with immediate vesting, and health benefitsOak Street Health is an equal opportunity employer.
