Promotes teamwork through mutual respect for co-workers, developing positive interpersonal relationships and lending assistance as needed.Position SummaryThe Nursing Program Manager works in concert with the business unit directors of The Palliative Care Clinic.  This individual is responsible for organizing and coordinating the day-to-day operation of Palliative Cancer Patients with Opioid/Substance use Disorders.  The position will be supervised by the Palliative Oncology Outpatient Medical Director and Nurse Manager.   Duties include coordinating day-to-day clinic activities, providing patient education, tracking and coordinating ongoing patient care, coordinating and triaging patient care via phone and video, and collecting and reporting project outcomes.  In the performance of these assigned duties, the Harm Reduction Nurse Coordinator will assure adherence to protocols, procedures and program plans established for the Harm Reduction Clinic.   The Harm reduction Nurse Coordinator will assist in the care coordination of population groups or programs within Palliative Care to optimize patient care, safety and operational efficiency.Minimum Qualifications-     Required : Graduate of accredited school of nursing.  BSN , licensed to practice nursing in the state of Ohio,-     Master prepared strongly preferred-     Minimum of 2 years experience in nursing-     Demonstrated expertise in areas of health care quality, process improvement & patient safety.-     Strong communication, interpersonal, organizational and facilitation skills-     Knowledge, understanding and experience using performance improvement tools and methodology.-     Demonstrated ability to initiate and complete projects.-     Experience facilitating teams or committees-     Strong data management and analytical skills.-     Ability to organize data and information  into a succinct, effective and meaningful format for customers-      Computer skills, including basic knowledge and use of Microsoft Office products, electronic mail and the Worldwide Web/ Internet.
