Physical Therapist

Top Responsibilities Collaborate with interdisciplinary team members including nurses, PTAs, occupational therapists, and home health aides regarding patient care, updates, and concerns.Develop, implement, and follow individualized treatment plans in coordination with physicians and care teams.Provide in-home physical therapy services focused on improving patient mobility, strength, and overall functional independence.Monitor patient progress, reassess conditions, and modify care plans as needed.Maintain accurate and timely documentation of patient treatments and progress in accordance with company policies and regulatory standards.Participate in weekly case conferences and contribute to care planning discussions.Educate patients and caregivers on therapeutic exercises, safety techniques, and home care plans.
