You’ll be a key player on our care team, helping some of our most complex patients navigate their chronic conditions while ensuring they receive the highest quality care. You’ll work closely with Nurse Practitioners and RN Case Managers to coordinate care, facilitate resources, and keep the patient journey moving smoothly.Reporting to the Manager/Supervisor, this role will coordinate with providers and Integrated Care Nurses to provide ongoing support and communication to Chronic Kidney Disease (CKD) and/or End Stage Renal Disease (ESRD) patients. The Care Coordinator, Expert acts as a single point of contact to coordinate resources for DaVita IKC patients. This role acts as a team lead, is proficient at advanced escalations, acts as a resource to other teammates and peers and applies advanced critical thinking.Responsibilities:Performs outbound calls and responds to inbound calls from members, providers, and Village resources. Under guidance of Manager, completes non-clinical pathways & screenings; escalates to Integrated Care Nurse as needed.Helps members access eligible resources/programs and assists with the referral process and provider appointments on behalf of the DaVita IKC membersActs as a subject matter advisor to Teammates on navigating the member’s eligible health plan programs and other resourcesOther duties as assigned, such as mentoring/training new hires, leading special projects (e.g. process improvement), covering for TMs on PTO, and providing additional coaching to TMs as neededQualifications:High school diploma or GED required2+ years of experience in a professional office environmentAbility to handle confidential information with discretion and sound judgmentStrong verbal communication skillsProficiency in Microsoft Office, Excel, and OutlookPrior experience with Electronic Medical Records preferredCommitment to service excellence and patient satisfactionAbility to work autonomously while collaborating effectively with the teamHome workspace must provide a quiet, private environment with reliable high-speed internetPreferred Qualifications:1–3 years clinical medical office experienceCNA or MA experience is a plusExperience coordinating care (referrals, care management, etc.)Familiarity navigating insurance payersExperience in direct patient care or supportPrior medical office environment experienceWhat We’ll Provide:More than just pay, our DaVita Rewards package connects teammates to what matters most.
