You will work in a primary care clinic setting in partnership with a multidisciplinary team and receive referrals from primary care. You will collaborate with care team members to identify, address and treat mental health needs of patients within the Allina Health system. You will have a team at your home clinic as well as a team of Mental Health Consultants located at other Allina Health clinics.Key Position Details:The job responsibilities include individual therapy, diagnostic assessments, collecting and evaluating information about client/patient situations, establishing and updating treatment goals, and treatment planning.  Highlights of this role:  40 hours of frontloaded Paid Time Off  $6,000 retention bonus   Competitive salary and benefits package  An established environment that safeguards provider well-being.  Career support in a variety of ways, including monthly continuing education opportunities, and tuition and CME reimbursements   Job Description:Provides solution-focused therapy to patients independently and/or in conjunction with mental health provider. Provides crisis consultation and de-escalation to patients in person and/or virtually Collaborates with Primary Care/Women’s Health teams and community  Supports patients and primary care team/mental health providers in achieving patient care goals. Reinforces provider communication about plan of care. Coordinates care for patients across the primary care, specialties, and mental health service lines Assesses conditions related to crisis and life events. Triage patients with known mental health issues and provide same-day and short-term ongoing access to mental health therapy Screens for basic health needs and align services to assist. Consults, refers and coordinates interventions in cases of suspected or actual situations of child or adult abuse and neglect or forms of domestic violence or sexual abuse. Identifies needs for community or public agency services Job RequirementsMaster's degree in Social Work required2+ years in licensed practice preferredLicensed Ind Clinical Social Worker - MN Board of Social Work requiredBLS Tier 1 - Basic Life Support - Multisource preferred within 60 DaysPhysical DemandsSedentary:Lifting weight Up to 10 lbs. occasionally, negligible weight frequentlyPay RangePay Range: $35.61 to $48.81 per hourThe pay described reflects the base hiring pay range. Your starting rate would depend on a variety of factors including, but not limited to, your experience, education, and the union agreement (if applicable).
