Our centers of excellence include Baptist MD Anderson Cancer Center, Baptist Heart Hospital, Baptist Neurological Institute and Wolfson Children's Hospital. The Community Health Worker (CHW) position is responsible for acting as a bridge between health care providers, community services providers, and individuals in the community to promote health, reduce disparities, and improve service delivery. The Community Health Worker is a trained and trusted public health worker who is respected by the people they serve and applies his/her unique understanding of the experience, socio-economic needs, language and/or culture of the communities served.  As part of the care team, the Community Health Worker provides the additional human resources needed to ensure patients are served within a primary care medical home, receiving the health education needed to address chronic conditions like diabetes, cardiovascular disease, COPD, and others that impact quality and years of life.Primary Location:Downtown - 841 Building
