Description:Hospice RN Case ManagerMon-Fri 8-5p, and 1 weekend a month. no on call.
Covering Fresno
The Hospice RN Case Manager is responsible for delivering comprehensive, patient-centered nursing care to hospice clients. This role serves as the primary coordinator of care, ensuring patients receive appropriate symptom management, emotional support, and end-of-life care aligned with physician orders and hospice plans.
Key ResponsibilitiesClinical Care & Case ManagementPerform patient assessments, develop and update individualized care plansManage overall patient care, serving as the designated case managerMonitor patient condition and communicate changes to physicians and care teamProvide symptom management using evidence-based hospice/palliative practicesDeliver education to patients and families on disease progression, self-care, and end-of-life expectationsCare CoordinationCollaborate with interdisciplinary team (IDT/IDG), including physicians, aides, and other cliniciansCoordinate referrals and services to ensure timely, efficient careLead case conferences and contribute to care planning discussionsSupervision & LeadershipProvide functional supervision to LPNs and Hospice AidesConduct supervisory visits (bi-weekly for aides, monthly for LPNs)Delegate tasks appropriately based on skill levelParticipate in training, orientation, and mentoring of staffDocumentation & ComplianceMaintain accurate, timely clinical documentation (assessments, care plans, visits, physician communications)Ensure compliance with federal/state regulations, hospice standards, and agency policiesUphold confidentiality and HIPAA standardsPatient & Family SupportAddress grief, loss, and emotional needs of patients and caregiversProvide anticipatory guidance around the dying processPromote dignity, comfort, and quality of lifeMinimum QualificationsRN license in state of practiceGraduate of accredited nursing programCPR certificationMinimum 1 year acute care nursing experienceHospice/home health experience preferredStrong communication skills, especially around grief/lossValid driver’s license and reliable transportation(California-specific: 1 year professional nursing experience required)
