The role builds strong relationships with families to identify goals, strengths, and needed services through home visits, assessments, and ongoing follow‑up. It supports families in accessing resources such as health care, crisis assistance, education, counseling, and employment training, while also helping them engage in their child’s development and advocate during transitions. The position provides health, nutrition, and safety information to parents and assists with medical and dental follow‑through. It also guides staff on ERSEA and health requirements, actively recruits families to maintain full enrollment, and promotes consistent attendance. Additionally, the role collaborates with community organizations to expand access to health, mental health, nutrition, and disability services, and offers recommendations to leadership to support continuous improvement.Key Job Responsibilities:Builds partnerships with parents to identify family goals, strengths, necessary services, and other supportsRefers families to services and resources, including emergency/crisis assistance, accessing on-going health care, education, counseling, and employment training, and services.
