Department:36102 Advocate Christ Medical Center - Emergency DepartmentStatus: Part timeBenefits Eligible:NoHours Per Week:16Schedule Details/Additional Information:32 hours per pay period, rotating shiftsPay Range$24.10 - $36.15Major Responsibilities:Coordinates and conducts post discharge follow-up telephone calls with patient or family members.1)Seeks list of previous day patient discharges from system administrator and attempts to contact patients by telephone.2)Reinforces and or clarifies patient teaching and discharge plan in accordance with the discharge instructions as provided by the physician and RN.3)Seeks information regarding the patient’s satisfaction with their ED visit and staff that should be recognized performing service recovery, as necessary.Coordinates physician notification of culture results for discharged ED patients.1)Conducts daily review of laboratory culture results for discharged ED patients2)Reviews culture results and evaluates the antibiotic treatment choice for appropriateness based on the sensitivity indicated on the report.3)Seeks council of ED attending physician when questions arise.4)Distributes reports, as appropriate, to an ED attending or the primary care physician who will communicate the results and any revision of the discharge instructions to the patient.Monitors department patient care equipment in a state of readiness.2)Performs daily check of the crash cart and monitor located in the ED Radiology Holding Area and logs results in the logbook.3)Notifies Biomed and MCO/designee of malfunctioning portable monitor(s) and removes it from service.4)Identifies any missing portable monitors and seeks their return.1)Performs daily checks on all portable monitors, including crash cart monitors and logs results in the logbook.Utilizes communication skills, promotes multidisciplinary coordination, and cooperative working conditions.1)Uses verbal and non-verbal communication skills to identify and reduce patient/family anxiety.2)Collaborates with other health team members to reinforce discharge instructions and to ensure patient privacy and confidentiality.3)Participates in patient satisfaction initiatives to promote a positive ED experience.4)Serves as liaison for family members in the ED Reception Area, as requested.Demonstrates commitment to the department goals, organizational goals, and to the standards and practices of all regulatory agencies.1)Participates in department meetings and activities.2)Attends to personal work schedule in a consistent and professional manner according to established departmental guidelines.3)Facilitates ED special projects as directed to assist achieving department goals and enhancing patient services.Education/Experience Required:•Graduated from accredited Practical Nursing program. •3 year of prior experience as an LPN.Knowledge, Skills & Abilities Required:•Ability to cooperate and work with others. •Good communication skills. •Familiar with PICIS computer patient documentation systemLicensed Practical Nurse in the State of Illinois with current licensure completion of approvalPhysical Requirements and Working Conditions:•Ability to work weekends, holidays and different shifts in order to accommodate staffing needs. •Ability to lift and/or move patients up to 100lbs may be requested. •Ability to work under stressful conditions and in difficult situations. •May be exposed to hazardous materials and life-threatening diseases. •Ability to perform lifting/transfer activities related to patients. •Ability to work rotating shifts any day of the week.If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available.
